If you like what you hear, book your 45min 1:1 Online Initial Assessment. This is a detailed assessment to get to know you, your activity background, medical history, dive into your personalised needs, and direct how form.at.physio can support your journey.

Complete online forms
(via data protected email link)

 

Once your initial assessment booking is confirmed, you shall receive a series of online forms from Form.at.Physio to your email address, including: Onboarding information, Health Screening form, and Consent forms to receive online physiotherapy as well as GDPR Information about how your data is stored. Please note, we will need all these forms completed before we can start our first official session together.

Healthcare security is very important to Form.at.Physio and our secure telehealth platform prioritises privacy and professional standards. To protect you and your medical data we use a practice management software called 'Jane.app'. Your data will be processed and stored in London, UK, adhering to UK GDPR procedures.

Treatment & Exercise Plan

Following your initial consultation, form.at.physio will design a personalized treatment plan aligned with your movement goals. This plan will include strength and conditioning, education, stretching, and tailored programming recommendations. During your one-on-one online session, we will demonstrate the exercises, observe your movements, and provide real-time feedback and adjustments as needed. Your rehab plan, including supportive exercise videos, will be delivered via email/ whatsapp within 24 hours of your appointment.​

Follow-up sessions and ongoing support

Online physiotherapy usually involves a series of follow-up sessions to track your symptoms and progress your exercise program using the latest evidence. Form.at.physio will provide a personalised programming schedule designed to align with your goals. You can easily book follow-up appointments online through your 'Jane.app' patient portal. For a session series, consider upgrading to our Longevity Program.

What is online physiotherapy/ remote consultation?

Online physiotherapy (also called telehealth, tele-physio, virtual or remote consultation) is a real-time assessment and treatment session conducted via secure video or telephone, instead of in-person. It includes evaluation, tailored advice, education, exercise prescription, progression and monitoring delivered digitally.

Yes. Physiotherapists must be registered with the Health and Care Professions Council (HCPC) and adhere to professional standards by the CSP whether working digitally or in person. Remote consultations must comply with confidentiality, consent and clinical safety requirements as part of clinical governance.

What conditions can be effectively managed online?

Most common musculoskeletal (MSK) conditions — such as back, neck and joint pain, post-surgery rehab planning, movement dysfunction and postural issues — can be assessed and managed online through guidance and exercise prescription. Physical hands-on treatments (e.g., manual therapy) are limited remotely, so some conditions may require in-person follow-ups.

How do I prepare for an online physiotherapy appointment?

 

You’ll generally need:

  • A quiet, well-lit space

  • A device with webcam & microphone (phone, tablet, laptop)

  • Stable internet connection

  • Comfortable clothing to demonstrate movement

Are my online consultations private and confidential?

 

Yes. Form.at.Physio online sessions are conducted via Jane.app a secure, UK GDPR-aligned platform and treated with the same confidentiality and data security as face-to-face sessions. Our Physiotherapists protect patient information at all times.

Do I need a GP referral for online physiotherapy?

 

In private practice, you often do not need a referral, you can self-refer directly for a Form.at.Physio consultation.

How will a physiotherapist assess me without touching me?

 

Remote physiotherapists use detailed subjective history, observation of movement, biomechanics, posture and functional tasks via video to make clinical decisions. If a thorough assessment is not possible remotely, they may recommend face-to-face follow-up.

What happens if my condition is complex or doesn’t improve online?

 

If your condition requires hands-on examination, manual therapy or specialist equipment, the therapist will advise an in-person appointment or refer you onward. A safe escalation pathway is part of good practice.

Can family/carers join my online physiotherapy session?

 

Yes - with your consent, a carer or family member can join the session to support communication and help with exercises at home.

Private health insurers sometimes cover remote physiotherapy, but coverage varies by provider and policy. Always check directly with your insurer before booking.
